A Death Has Occured

Step 1: Identify the appropriate scenario

A Death Has Occurred at Home or Assisted Living:

  • CALL 911
  • Advise 911 operator IF deceased has a MOLST or Out-of-Hospital DNR. 
    [Note: You must have document(s) in your possession.]
  • DO NOT CALL 911 IF UNDER HOSPICE CARE! Contact your Hospice Provider.
  • Call Kleinberg Funeral Services AFTER calling 911.

A Death Has Occurred at a:

A Death Has Occurred and Deceased Was Removed to NYC Office of Chief Medical Examiner (OCME) or County Medical Examiner Outside of NYC.

The First Call

Step 2: Call the funeral home

Below are some of the questions we will ask upon initial contact:

  • Name of Deceased
  • Location of Deceased
  • Date & Time of Death [If Known]
  • Personal Physician’s Name & Telephone Number
  • Hospice Provider
  • Caller’s Name, Phone Number & Relationship to Deceased
  • Person Legally Authorized to Make Arrangements
  • Do We Have a Prearrangement on File
  • Is the Deceased a Veteran
  • Desired Arrangements
  • Permission to Move Deceased

Vital Statistics

Step 3: Provide us with vital statistics

We are required to file the death certificate in the jurisdiction where death occurred prior to services and/or disposition of the deceased. We rely on you to provide us with accurate information. Below are the required items--you may provide them at the time of the first call, during the arrangement process or via our online Vital Statistics Form.

  • Decedent’s Legal Name
  • Decedent’s Usual Address

  • Social Security
  • Usual Occupation
  • Kind of Business or Industry
  • Aliases or AKAs
  • Birthplace
  • Highest Level of Education Obtained

Step 4: Addresses your specific prefences

  • The process of arranging a funeral varies by your unique situation--during our initial phone contact we will discuss how to proceed based on your comfort level.
  • We find most families prefer to communicate utilizing a combination of platforms: Phone, Text and Email, and to a lesser degree Face Time and Zoom.
  • We will gladly meet you in person: Whether it’s in the comfort of your own home, the funeral home, or your favorite coffee shop, we do our best to accommodate you.
  • Our casket selection room is 100% virtual: Whether you are located in Island Park, Long Island or the island of Manhattan, you can view an array of more than sixty caskets online. We will gladly guide you through the selection process so you can make an informed decision.
